What documents do you need to apply?

To apply for your Kentucky Medical card, you’ll need to complete the application available on the Kentucky Medical Cannabis Program website. The application will require specific personal information such as your full name, address, telephone number and a driver license or state issue ID to ensure you are a Kentucky resident.
